Never auto-approve destroy-class breakpoints overview
Mirrors a BreakpointRule with action=never-auto-approve covering the `destroy` ActionCategory (permanently deletes / overwrites state). The default ApprovalPosture for destroy-class actions sets allowAutoApprove=false and requireExplicitRule=true, so this rule forces every destroy breakpoint into the human-review queue regardless of consecutive-approval streaks.
